Please observe these simple precautions before your office visit. We welcome your traffic and appreciate these modest concessions to efficiency and general goodwill between health care provider and patient.
Appointments and Renewals for medications must be made during office hours, including the long days and excluding the Wednesday off day.
Requests for non emergency medical care and answers to questions, must preferably be made early in the day to allow more time for a reply.
Referrals must be requested at least 4 office days prior to the appointment made.
Waiting room priority is set by time of appointment, but may be altered by urgency of medical need.
As billing for care to insurance providers becomes increasingly complex and time-consuming, we offer discounts to patients who pay by cash or check.
A 48 hour notice of non availability for an appointment is requested to leave room for others. A fee may be charged for repeated no show or without anough time prior to cancellation.
Request for copies of medical records by a person or by insurance companies, must be appropriately signed and presented to the staff, by a responsible person of major age, with an accompanying reason, and cognizant of fees that may accrue.
